RQS CIW Access
In order to run any RQS query, you must have CIW Access!

If you are looking to access the RQS Queries, then please fill out the following forms completely and return them to the registrar's office.

CIW Access Form

CIW Access Form for Non-University Affiliates

Return the following form to the Office of the Registrar either by fax or by campus mail (see below).  All forms must be fully completed or it will be returned to you.  This form will allow you access to the Central Information Warehouse (CIW) and ultimately the RQS General Queries.  If you choose to request a custom built RQS query, there is no need to fill this form out again, your access will work for both your custom queries and the general queries.
