HB1023-Lawful Presence - How to Submit a Form

There are two options for submitting the HB1023-Lawful Presence Form:

Visit the Office of the Registrar during normal business hours.

Staff members will provide you with the form and verify your identification. Please make sure you bring the accepted identification with you. The list of accepted identifications can be found in the FAQ section.

You may fax or mail a notorized HB1023-Lawful Presence form to the Office of the Registrar.
